Starting care is simple — and we’ll work for and with you every step of the way.
Apply for Medical Assistance
Contact your local county human services office to apply for Medicaid (Medical Assistance) benefits.
Request a Nurse Assessment
After applying for MA, contact your county or health plan to have a nurse come authorize services. Let the nurse know you’d like Heart & Hara to provide your continuum of care.
Already Have Services or a Caregiver?
If you have existing services or a caregiver from another agency, contact our home office — or submit our Client Referral form and employee packet online.
New to Home Care?
If you’re new to home care or don’t have a caregiver, we’ll help you secure a responsible, reliable, and trusted caregiver. We go above and beyond — we’ll advertise and recruit on your behalf!
